This privacy policy applies to Hydrotherapy Studio, operated by Koaben Pty Ltd (ACN 603 302 450, ABN 40 603 302 450), located at 5/54 Kingston Rd, Underwood QLD 4119. We collect personal information that is necessary to provide our physiotherapy and hydrotherapy services. This includes your name, contact details (phone, email, address), date of birth, health history and medical conditions, referral and funding details (NDIS plan numbers, Medicare numbers, WorkCover claim numbers, CTP claim details, Home Care Package provider information, private health insurance details), payment information, and booking records. Health information is classified as sensitive information under the Privacy Act and is collected with your consent as part of your physiotherapy assessment and treatment.
2. How We Use Your Information
We use your personal information to provide physiotherapy and hydrotherapy services, process bookings and appointment reminders, bill funding bodies including NDIS, Medicare, WorkCover QLD, CTP insurers, Home Care Package providers, and private health insurers, communicate with you about your treatment, comply with our obligations under AHPRA registration requirements and relevant health legislation, and improve our services. Your health information is used solely for the purpose of providing your treatment and meeting our legal and funding obligations.
3. Disclosure of Information
We may share your personal information with your referring GP, specialist, or other treating practitioners (with your consent), NDIS plan managers and support coordinators for billing purposes, Medicare, WorkCover QLD, CTP insurers, and Home Care Package providers as required for claims processing, payment processors for transaction processing, and government bodies or regulators as required by law. We do not sell, rent, or trade your personal information to any third party.
4. Data Security
We take reasonable steps to protect your personal information from misuse, interference, loss, and unauthorised access. Your health records and personal data are stored securely in our practice management system (Splose), which uses encryption at rest and in transit. Access to your records is restricted to your treating physiotherapist. Health records are retained in accordance with Queensland Health records retention requirements — a minimum of 7 years from the date of last service, or until the patient turns 25 (whichever is later) for records of minors.
5. Your Rights
Under the Australian Privacy Principles, you have the right to access the personal information we hold about you, request corrections to any inaccurate or out-of-date information, and request deletion of your information where it is no longer needed (subject to our legal retention obligations).
